You expressly consent to our processing of the following for the purposes stated:

• Application data, including identity, contact, training background, and optional motivation text
• Health-related information you voluntarily provide about injuries, pain, surgeries, or musculoskeletal conditions relevant to safe training adaptation
• Workout video recordings and audio captured during the Beta Program as instructed
• Movement data derived from recordings, including pose estimates, biomechanical metrics, labels, and quality scores
• App usage data, diagnostics, and feedback you submit during the cohort
• Use of recordings and movement data to train, test, and improve movement recognition algorithms and neural network models
• Use of data for coach review, quality assurance, safety monitoring, and product development
• Sharing with authorized coaches, engineers, and processors bound by confidentiality and data protection obligations
• Retention as described in the Beta Privacy Notice

Health-related answers and movement recordings may constitute special category or sensitive data under applicable law. Your explicit consent is the primary legal basis for this processing during the beta.

Do not upload medical records, clinical imaging, or information unrelated to exercise participation. Provide only what is necessary for safe coaching adaptation.

You understand and consent that recordings and derived movement data may be used to develop and improve machine learning and neural network systems that power technique analysis and coaching features.

Such data may be reviewed by qualified personnel for labeling, validation, and error analysis. We implement access controls designed to limit use to authorized program purposes.

Your consent is voluntary. However, certain processing is necessary to deliver the Beta Program. Without consent to essential processing, we cannot enroll you or continue your participation.

Optional fields in the application remain subject to separate consent selections where indicated.

You may withdraw consent at any time by emailing info@ohmyperfectform.com with a clear request. Withdrawal does not affect the lawfulness of processing before withdrawal.

If you withdraw consent for processing essential to the program, we may need to suspend or terminate your participation and delete or anonymize data where no other legal basis applies.

You may exercise applicable data subject rights as described in the Beta Privacy Notice, including access, correction, deletion, restriction, and objection where permitted by law.

This Consent must be read together with the Beta Privacy Notice and Beta Participation Agreement. In case of conflict regarding data processing, the most specific data protection document prevails.

By selecting the consent checkboxes in the application or signing onboarding materials, you confirm that you are at least 18 years old, have read this Consent and the Beta Privacy Notice, and agree to the processing described herein.
